Growing up in a bed and breakfast environment, my daughter hangs out with a LOT of adults. She loves playing with other little kids at the park and museums, but for the most part she spends most of her days hanging out with adults.
She does have one very good toddler friend, Madelyne. It’s no surprise that Madelyne’s parents happen to be me and my husband’s close friends. What many people don’t realize though, is that Cicily and Madelyne were born just 2 hours apart. They are what we call, soul sisters.
We just celebrated their two-year-old birthday party together earlier in the month. I know it’s cliche but its amazing how fast the time goes by! Now that they are each coming in to their own personalities, I wanted to find a way to remember just how hilarious they both are at this age.
I’ve seen these birthday questionnaires floating around on Pinterest, so I thought it would be fun to make up my own set of questions for the two girls to answer each year. I jotted down the questions I liked best and made this birthday questionnaire for them to answer each year. I also included a drawing section where they could draw a picture of what their families looks like each year. Being that they are only two, there are mostly just scribbles there…but they are the cutest scribbles you’ll ever see. 😉
Once I found some colorful books with blank pages at Target in the dollar goodie aisle, I was all in. I created two little books so we wouldn’t forget to fill out the birthday questions each year.
I’ve uploaded the free printable pages for you to download and use to create your own book. You just need to print one page for each year you want them to fill out. I made enough for years 2 through 16. Then glue the questions to a blank page in a book or insert into a plastic sheet protector / folder combination.
What you need:
- Blank book or folder with sheet protectors
- Paper and printer
- Glue stick
- Name and Birthday Cover template (download here)

Step 1 – Print out the cover and questions template by clicking the images below. You will only need one cover sheet. However, print one questionnaire for each birthday or year you’ll be asking your child to answer the questions.

Step 3 – Glue the cover page in to the book of your choice with a glue stick.
Step 4 – Glue the questionnaire pages in to the book with a glue stick. We used enough to cover ages two through sixteen, but you could do it for whatever amount of years you’d like!
Step 5 – Each year on their birthday, have your child answer the questions and draw a picture of their family.
I made one of these birthday questionnaire books for Madelyne and her mom to fill out each year. These are such fun gifts to give that make each birthday special!
I think these are adorable as baby shower gifts to make for the mom to be. You could even turn it in to a little game or activity at the shower where you collectively create the list of questions to ask each year.
